Living Will/Advanced Directives Will offer guidance and support in completing the Five Wishes Form, a living will document. Five Wishes is written in everyday language and helps people express their wishes in areas that matter most — the personal and spiritual in addition to the medical and legal. It also helps you describe what good care means to you, whether you are seriously ill or not. I have these forms available in English and English/Spanish. A living will form, Voicing my Choices, for adolescents and young adults who are living with a chronic illness or are approaching end of life is available as well so they too can make empowered decisions about their care, and what they want to happen after they die. Initial consultation During a 60 minute consultation we will discuss options to support the dying person, funeral services, and body disposition.
Vigil Sitting with the dying person, creating a peaceful and nurturing environment, and giving respite to the primary caretakers at home, hospice, or hospital.
Facilitation of home funeral Overseeing and offering support, educating the family about ritual body care, and preparation of space.
Support with funeral home Accompanying the family to meetings at the funeral home, facilitating communication with the funeral home director, arranging transportation, wake, and body disposition, etc.
Officiating Funeral Service/Memorial Service Creating a personalized service/ceremony in alignment with the beliefs and vision of the dying person or their designated proxy.
Energy Healing Session Providing energy healing for the dying person and/or caretakers in home, hospice, or hospital.
